Description of the project idea: 
This project aims to develop and implement a shared expertise collaborative training model utilizing service design methods. The outcome is to have a set of various alternative primary healthcare solutions developed togehter by healthcare professionals and experts by experience trained in this project, to meet the needs of vulnerable groups in the population in Baltic area. In societal context, this project aims to create more cost-effective and high-quality healthcare services and increase well-being social inclusion and health promotion within the whole population.
